To request new service, change existing service, or disconnect service:

• Duke University and Hospital: Download, print, and complete the pager request form. Fax the form to the OIT Service Center at (919) 613-6476 or visit the OIT Wireless Service Center located in Duke South near the Medical Center Bookstore. For small orders of new pagers, please allow up to one business day for delivery to the OIT Wireless Service Center. For orders of 20 or more new pagers, please allow up to five business days for delivery. Office hours are Monday - Friday, 8 a.m. to 5 p.m.
• Durham Regional Hospital and community physicians: Complete the online form.
• Duke Health Raleigh Hospital: Complete the online form.
